Where is the balance between giving someone an ”out” and holding them accountable?

Creating accountability can be awkward and it's sometimes easier to just say "oh it didn't work out." But letting people take advantage of you isn't okay either.

How do you decide when to put your foot down? My experience is that most people wait too long.
